Social Drivers of Health and Health-Related Social Needs Resources for Blue Cross NC Patients

The following websites may be used to identify available, local, resources and assistance programs for Blue Cross NC patients with an identified social drivers of health (SDOH) and/or health-related social needs (HRSN).

  • The following toll-free phone lines are available for Blue Cross NC patients to call and speak directly with a Care Manager who can help connect them with community resources that offer additional support:

    • Members with Commercial Blue Cross NC Health Plan:

      • Call: 1-800-218-5295

      • Hours: Monday-Thursday: 8 a.m. – 7 p.m. ET | Friday: 8 a.m. – 5 p.m. ET

    • Members with Blue Medicare Advantage Health Plan:

      • Call: 1-877-672-7647 (TTY: 711)

      • Hours: Monday-Sunday: 8 a.m. – 8 p.m. ET

  • Please see attached flier at the bottom of this page about Blue Cross NC’s Case Management Program which may be printed and handed to patients upon identification of SDOH-related barrier and/or HRSN.

  • FindHelp is a free platform that searches through Community Link's database of free or reduced cost services.

  • You may browse available services by ZIP, keyword, program name, or even specific categories such as food, housing, goods, transit, health, money care, education, work, or legal.

  • NC 211 is a free, information, and referral service provided by United Way of North Carolina and powered by local United Ways of North Carolina. Families and individuals can call 2-1-1 or 1-888-892-1162 to receive free and confidential information on health and human services within their community.

  • You may browse available services by zip code or topic: Basic Needs, Consumer Services, Criminal Justice and Legal Services, Education, Health Care, Income Support and Employment, Individual and Family Life, Mental Health and Substance Use Disorder Services, Environment and Public Health/Safety, Organizational/Community/International Services.

  • NC MedAssist, is a statewide free pharmacy program, which provides prescription medications to uninsured, low-income patients. Patients must live in North Carolina, have no access to insurance, and meet certain income requirements. Patients can see if they qualify for assistance by going to their website or by calling 1-866-331-1348.

NC Division of Health and Human Services (NCDHHS)

  • Linked here is NCDHHS Assistance Page which provides links to resources specific to children, disability, low income, hurricane recovery, and specialized care (e.g. Division of Aging, Reproductive Health Services, Mental Health and Substance Use Disorders, Veteran's) services.

  • Linked here is the Division of Public Health list of local health departments in the state linked with each department's website and phone number. Services offered can differ across health departments.

  • Individuals can call DHHS Customer Service Center's 1-800-662-7030 hotline to find program(s) and people to help.

  • For patients who have an employment-related need, NCWorks utilizes ZIP to search for local NCWorks Career Centers and job listings.

  • For veterans who may need additional benefits and services. Services are provided free of charge and access to current federal, state, and local veterans’ programs, entitlements, and referral services are available via in-person at a Veterans Service Office near the member or 24/7 access online.

  • Linked here is an interactive NC map of State Service Centers and County Veterans Service Offices. State Service Centers and County Veterans Service Offices offer personal assistance to veterans, military, and their families. The DMVA has a staff of qualified benefits specialists trained to help veterans and their family members obtain the benefits and services.
