Please use the following help article as a guide in completing Diabetes and Hypertension Introduction encounters.

  • Confirm correct information by selecting the checkmark icon

  • Remove inaccurate information by selecting the "x"

  • Add correcting information by selecting the "+" opening a new modal with fields corresponding to the respective data element

Open Task & Confirm Demographic Information

The first step in the Diabetes & Hypertension MTS tasks are to open the task and confirm the patient's phone number and address. Maintaining an accurate contact record will be critical to your ongoing engagement with members.


Task Assessments

Next, you'll perform assessments characterizing the patient's baseline disease state control, including history of present illness and baseline lab values.

Goals, Interventions, & Patient Print Out

Based on your clinical assessment, establish goal lab and vital benchmarks with the patient that you believe can be reached at the conclusion of their sequence of MTS encounters. Then move on to the next section were you'll indicate any education and interventions performed with the patient. In the resources section, you'll see both patient resources and homework items which you'll open and print as patient takeaways. Documents can also be downloaded as pdf's and sent to the patient via DocStation Conversations.


Finalizing Encounter Details & Completing Task

Document the type of outreach performed with the patient, along with the amount of time spent preparing for the visit and performing the encounter. Lastly, schedule the patient for Visit 2 of the MTS program 4-6 weeks following the date of service for Visit 1.

Complete the task by selecting the blue "Continue" button at the bottom of the task modal. You'll be prompted to generate any desired documents, such as an eCare plan for CPESN pharmacies.
