UCare MN CMR Program
Use DocStation to document Comprehensive Medication Reviews (CMRs) for your UCare patients and generate the required patient takeaway documents and CCD files.
Overview
The UCare MN CMR task allows pharmacists to document CMRs for UCare members and generate the Medicare-standard patient takeaway documents required for program compliance. Upon task completion, you can also generate CCD (Continuity of Care Document) files for UCare documentation requirements.
For CPESN Minnesota members: DocStation can be configured to automatically transfer CCD documents to CPESN for UCare program payment. Contact DocStation Support to request this configuration.
Getting Started
Before you begin: The UCare MN CMR program is opt-in. Your pharmacy must be enrolled in the program before the UCare CMR task will be available. To request enrollment, contact DocStation Support via Chat or email support@docstation.co.
Creating an On-Demand UCare CMR Task
Navigate to the patient's profile
Click the blue plus button in the top right corner to add a new task
Under the UCare header, select CMR
Click Add and Open to begin the task immediately
Similar program headers are available for BCBS MN and HealthPartners CMRs. The UCare header helps DocStation distinguish UCare CMRs from other CMR encounters, which is important for reporting and CCD routing.
Completing the CMR Task
Complete the CMR task as you would any other task in DocStation:
Review patient information - Verify the patient's medication list, conditions, and allergies are up to date in their health record
Document the encounter - Complete all required CMR fields including medication therapy problems identified, interventions recommended, and follow-up actions
Add clinical notes - Document any relevant clinical observations or patient preferences
Click Continue - Once your documentation is complete, click the Continue button to proceed to the Generate Documents modal
Generating Documents
After clicking Continue, the Generate Documents modal will appear with options for creating your CMR documentation.
Available documents include:
CMS Cover Letter - Required cover letter with provider contact information
Medication Action Plan (MAP) - Summary of medication therapy problems and recommended actions
Personal Medication List (PML) - Complete list of the patient's medications
CCD - Continuity of Care Document for UCare reporting requirements
You can also select an eCare plan from the dropdown menu to generate all documentation at once.
Complete any necessary fields to finalize the documentation.
Optional: Create a claim from this task
If you select the option to create a claim, DocStation will generate a medical claim to be submitted for the CMR encounter. This is not a requirement of the UCare program, but provides a convenient way for any pharmacy to bill for the CMR.
Once you have made your selections, click Continue to complete the task and generate your documents. The generated documents will be attached to the completed task at the bottom of the task view.
CPESN Minnesota Members: Automatic CCD Transfer
If your pharmacy is a CPESN Minnesota member and you would like DocStation to automatically send CCD documents from UCare CMRs to CPESN for program payment, you must request this configuration separately.
Important: This configuration is not enabled by default. Some Minnesota pharmacies have existing workflows for managing their own UCare CCDs, so automatic transfer to CPESN is only set up upon request.

Request that your organization be configured for UCare CCD transfer to CPESN
Our team will enable the configuration for your account
Once configured, CCD documents and eCare plans generated from UCare MN CMR tasks will automatically be sent to CPESN in near-real time once the task and documentation is complete.
Frequently Asked Questions
Q: How is the UCare MN CMR different from other CMR tasks?
The UCare MN CMR task functions the same as other CMR tasks (like HealthPartners or BCBS MN CMRs) but uses the UCare program header. This helps DocStation route documents appropriately and distinguish UCare encounters in reporting.
Q: Do I need to do anything special to bill for UCare CMRs?
Billing for UCare CMRs is handled according to your agreement with UCare. You can optionally use the "create a claim" feature during document generation to submit a medical claim through DocStation, but this is not a program requirement.
Q: What if I already have a workflow for sending CCDs to UCare?
If your pharmacy has an existing process for managing UCare CCDs, you do not need to enable the CPESN transfer configuration. Simply generate and download your CCD documents as needed and continue using your current workflow.
