Pre-Visit Survey
Launching mid-January, 2026, the Social Determinants of Health (SDOH) pre-visit survey will become an embedded questionnaire in Diabetes and Hypertension Visit 1a tasks in DocStation. The survey will include updated questions addressing key SDOH domains and will generate a score in real-time to assess the barriers to care and social needs identified. Within the survey, resources are available to provide immediate Blue Cross NC Care Management support as needed. When a social need is identified in the updated survey, a direct Blue Cross NC Care Management referral occurs within one month of survey submission for non-urgent matters. No additional action is required from the pharmacy following submission for the referral process to take place.
The pre-visit survey can be completed with the patient prior to engaging with the pharmacist and may be conducted by a pharmacy technician or student intern via phone in advance.
