The section below contain a number of files and resources that you'll see embedded within diabetes and hypertension tasks throughout your experience in "More Than a Script". You may access them here at any point or within the tasks when performing patient encounters.

💉Diabetes | Resources & Patient Printouts

Pre-Visit Survey

  • Effective January 1, 2026, the Social Determinants of Health (SDOH) pre-visit survey is embedded in Diabetes and Hypertension Visit 1a tasks in DocStation. The survey includes updated questions addressing key SDOH domains and generates real-time scores to assess the number of barriers to care and social needs identified. Within the survey, resources are available to provide immediate Blue Cross NC Care Management support as needed. When a social need is identified in the updated survey, a direct Blue Cross NC Care Management referral occurs within one month of survey submission for non-urgent matters. No additional action is required from the pharmacy following submission for the referral process to take place.

  • The pre-visit survey can be completed with the patient prior to engaging with the pharmacist and may be conducted by a pharmacy technician or student intern via phone in advance.
